  For this you need to leave both Jill and Rebecca behind, and then not fly off into the sunset at the end...
  To get this, just after you enter the room using the Spencer mansion key (to obtain the metal object) you'll get a cut scene where you'll hear Rebecca scream, simply ignore it and carry on to the courtyard.
  For Jill just don't bother collecting the MO discs, then you can't unlock the jail room in the lab.
  
  just to note you wont have to battle the Tyrant on the roof with this ending either.

  You can save Rebecca from the hunter, make a save file before the Tyrant fight, then let Tyrant kill her on the helipad. Just make sure to leave Jill in the jail cell. Reload and save them both for the other achievement. It's much faster and easier that way.

  I reloaded a save prior to Rebecca's encounter with the hunter, let the hunter kill her and left Jill behind. No achievement! Does it have to be a separate play through start to finish?

  It does not have to be a separate play through, both solutions will work, I personally was got this on real survivor mode when doing it, so didn't want to fight the tyrant on the roof with no auto aim etc. have you checked the achievement has unlocked but not popped?

  it does, when you need to help Richard or anyone, just ignore him, ignore everyone

  Here's how to get both "The Nightmare Ends" and "Every Man For Himself" achievements in one playthrough.
  
  Help Richard with Serum. (He will die inevitably from the Shark later)
  Protect Rebecca from Hunter. (Head to the room above when you hear her cry out)
  [SAVE GAME IN ANOTHER SLOT BEFORE RELEASING JILL FROM PRISON]
  Release Jill from prison.
  Protect Rebecca from Tyrant Helipad fight.
  Complete Game.
  
  "The Nightmare Ends" will pop.
  
  [RELOAD ALTERNATE SAVE SLOT BEFORE YOU RELEASED JILL FROM PRISON]
  Leave Jill behind in prison.
  Let Rebecca die by Tyrant on Helipad. (Make sure she actually gets grabbed and impaled to death)
  Complete Game.
  
  "Every Man For Himself" will pop.
