There are 10 Spartan Ops episodes of 5 chapters each. To unlock this achievement you need to complete all 50 chapters on heroic difficulty. This isn't as much of a pain as it looks as you have unlimited lives and anything you kill stays dead so you can brute force these.
The difficulty achievements stack so if you are going for them on legendary you will unlock all of the lower difficulty achievements when you meet the legendary requirements.
