So it sounds like they changed up the way the scoring works on Heaven or Hell. Thank you Tenkizuna for the info!

This did not work on "Heaven hell" difficulty for all levels, but I did obtain a few SSS ranks (levels 1, 2, 12, and 15) and a few SS ranks.

I think it's because of a patch that the solution doesn't work correctly anymore.

from a patchnote :
"Complete rebalance to style/time/mission pars, on all difficulty levels. This is important because it's one small point but a huge amount of rework"
So it sounds like this solution can still help you to achieve SSS in a few levels. Furthermore, you can combine difficulties and do a few levels on heaven or hell and the rest on another difficulty! Sorry this doesn't work for all levels anymore.

Basic idea:
This achievement is SIGNIFICANTLY easier you might think. As noted in the previous guide, it doesn't matter which difficulty you do the missions on. Therefore, you can do these missions on Heaven or Hell difficulty. The score requirements for Heaven or Hell seem to be different than the score requirements for the other difficulties. Most people just run through heaven or hell quickly shooting everything. The base score for shooting an enemy with your guns is 16 points, but the base score for hitting enemies with arbiter is much higher. Just run around and hit everything with arbiter and you will get more than enough points to get the SSS rank. You don't even really need to worry about getting your combo up with other weapons. Just use the basic attack with arbiter.

Other notes:
I pulled myself to the harpies with LT + x and then hit them with a rebellion basic attack. Harpies will just dodge out of your pull.

For the smaller enemies that fly in the air and shoot you I pull their shield away with RT + X and then pulled them to me and then hit them with arbiter.

Even the blue enemies that are normally only damaged by eryx and aquila attacks will die from arbiter. For the blue rage guys pull yourself to them and hit them with arbiter. To be safe you could jump in the air and then use RT + Y to throw arbiter at them, but I think this is actually fewer points than a basic arbiter attack (not sure though, the point values are so easy on heaven or hell that I didn't pay 100% attention to what was the highest value since smacking things with arbiter worked). For the blue knight guys just use your judgement or trinity smash combo. The first couple hits will knock their shield away for a second and leave them vulnerable.

I did this on missions 1,2 4,5, and 8-12. I also did mission 15 on the regular difficulty not realizing that heaven or hell would be easy. Mission 3 and 15 are probably also really easy on heaven or hell.

Of course, make sure you have all collectibles on a level before you try to get the SSS rank.

You can get hit on heaven or hell and it doesn't count as a death or an item used. It shows 3 golden orbs in the corner, but these don't count as lives used or items used. When you get hit a 4th time it will count as a death and decrease your score by 30%.

I'm honestly AWFUL at this game and even I managed to do this on heaven or hell. I had top 2-5 scores on most missions when I went to the leaderboards.

If you have any questions feel free to comment and I will respond.

JayD101 notes that each level only counts once, even if you do it on multiple difficulties.
