An interrupt occurs during combat when a character moves into the area of attack of a machine gun that has been set up by the other side and takes damage. When one of your characters is interrupted, you'll see the word "Interrupted!" appear in the battle report on the bottom right of the screen. You don't see that when your character interrupts an enemy, but you'll know when it's happening because you'll see and hear the machine gun firing, and you'll see and hear the enemy taking damage. The battle report calls it "Covering Fire" when you do it.

If you haven't progressed far enough in the story yet to know this, a machine gun is different from a tommy gun. You'll unlock the machine gun when you add Charles "Pops" Campbell to your gang. Unlike the handheld tommy gun, the machine gun must be set up on a stand before it can be fired. To get interrupts, you must leave the machine gun set up between turns.

The best way to get interrupts is to set up the machine gun to cover a doorway that a lot of enemies are going to pass through. I found the map "Flophouse Flush" on the fifth mission, "Bacharach Blvd.," to be ideal for this, because there's one doorway at the very beginning of the map that's between your characters and all of the enemies. Just keep your whole gang outside, set your gunner up covering the door, cause a commotion to alert the enemies, and wait for them to come out. After three enemies come through the door and get hit - which they usually do - the achivement will unlock.
