This has to be done in one challenge, without pausing. My first time through, I paused around 21 to recover and go back to it, only for the achievement to list as Done! Unlocking... By doing it a second time without pausing, it popped immediately after the 30th push-up.

Perfect is attained by going all the way down to the ground, then fully back up. While it doesn't matter if you do push-ups normally or on your knees, it's best to be angled towards the TV instead of perpendicular or parallel. I found it read me best at a 45 degree angle. If the game still isn't reading you, take a look at the tutorial for a quick refresher on proper form. Arms shoulder width, and if you do it on your knees, your feet still need to be on the ground.

Again, form is much more important than speed. While quick math dictates you need to do 0.333 push-ups per second, the answer is higher than that to account for the Freeze bonuses. Whenever I heard Freeze, rather than stop, I dropped straight down. Part of this is to rest, the other part is I had a nasty coincidence of getting OKs or Goods when the freeze wore off before I started doing anything. While this resulted in a little more work, as pushing back up at that point doesn't count as a push-up, it preserved the perfect streak.
