Missable achievement. You can only trigger this event once during the mission that sends you into the temple. If you miss it, you'll have to ratchet the difficulty up a notch to use the same character, or will have to use another character altogether.

If you must do this again and are cooping the game, it is much easier for one person to use new character on normal difficulty and the rest of the players bring their primary high-level character. TVH and UVH present an annoying challenge if your only goal is to obliterate the DLC as fast as possible to try this achievement again. Speed running the DLC's campaign on normal difficulty takes 2 hours to get back to the point where this achievement can be attempted again, so at least you're not looking at a significant time sink if you missed it the first time.

This achievement is either buggy or only granted to a one person at a time. With my coop partner as host and myself having triggered the statue, I was the only one to receive this achievement even though both of us outran the wheel. Elyoh contributes that the achievement is likely given to the first person who jumps out the door at the end of the corridor where you outrun the wheel.

Do not enter the VaultHunter.EXE node. Traversing into the VaultHunter.EXE node will take you past every area required for this side mission and achievement. On your first visit to Subconscious you must walk up to the entrance and then away again to retrieve items critical to the story mission, and at this point the remainder of the floating islands will pop up for you. If returning to this area after the story mission has been completed, just ignore the VaultHunter.EXE node entirely.

When you reach Subconscious, there will be a named location called Think Tank that contains a sidequest. This sidequest will take you to the Temple of Boom to get the achievement. Follow the white line eastward to reach Think Tank and obtain this side mission (optionally, the green paths are available if you come back to this area after completing the story mission, and are much faster/easier.)

You must first grab a disembodied claptrap eye (that the NPC refers to as a "gem") by traversing back from whence you came, west along the white line. Then, this claptrap eye must be taken to a claptrap statue on a cliff (that the NPC refers to as "the rod of putting") by following the white line east again, then following the red line south. Once the eye is placed into the statue, a temple opening then appear a short distance away where this achievement takes place.

Once you've dispatched the miniboss, jump up on the slab of concrete that the large Claptrap statue is holding and a mission critical item will appear as you fall into a hole below (this item has propensity to be buggy and may not appear in the same place on everyone's screen if you are coop-ing this). After picking up the item, a Claptrap will blather for a bit before the wheel behind him releases. A secondary mission objective appears that says "Outrun wheel", and you should be eligible for the achievement as long as you don't fail this objective. Turn around and just run to the end of the hallway, and a jump pad will appear outside as you draw near. This achievement should unlock right as you hit the jump pad.
