This is going to require multiple playthroughs to get. In your first run through the game, you will likely be able to get items from the first two columns of each type of gear. But after, that it will be very, VERY rare to see equipment. (This makes it much harder than the Rune collector since you can find all the runes in your first run through the game).

For the equipment, here are some things to look out for:

-Typically, the gear blueprint is in either an iron lined chest or the gold chests for defeating a mini-boss. These are more likely to contain the equipment blueprints. There are cases when a regular chest holds that hold the blueprints, but they are much less likely.

-As I said above, the gold chests for mini-bosses frequently hold blueprints. These rooms usually have a ? over them on the map so if you're using a Miner/Spelunker, that's how the room will appear on the legend.

-You need to speak to the blacksmith to get the achievement. The achievement will not unlock when you grab the last blueprint but after speaking to him when he has received them all.

I required NG+ and NG+2 to get the achievement. It might be possible to finish it on NG+, but I had done a speedy run of NG+ in order to finish 2 runs in one session and receive the "Beat the Game Twice" achievement. All in all, take your time and get the equipment. Really focus on exploring the castle and finding treasure chests throughout. Eventually, you'll get everything and finish the requirement for the achievement.

EDIT: According to Hikaru No Go, you can find items even in the 3rd and 4th columns during your first run through the castle. This might be because of reaching such a high level. Either way, it is possible to get very high level gear early (just might take a while).

EDIT 2: According to MemoriesOfFinal, getting Calypso's Compass will be a huge help. It can usually guarantee finding a blueprint in the secret room it guides you to. I personally did not use the compass, but this can help guide you to items such as blueprints or stat ups.

EDIT 3: According to ShotyMcFat, the region of the dungeon you are in will change what you find. Stronger gear will obviously be in the harder regions.
