Once you have built your first keep, you can place a couple in the keep as regents.
Eventually, as long as neither of them are infertile, they should bear children. You can increase the successful odds of this by pairing two very fertile characters together. It may take a few in game years, but once they have their first offspring this achievement will unlock for you.
