This achievement can be unlocked in the "Night Club Challenge" mode on the first available course, Wolf Creek.
You need to progress through the challenge mode, getting at least 1 star in each challenge to unlock the next. The first challenge with portals in it is the "Portals!" challenge, which can be found on the 5th hole, and is the 21st challenge overall. The objective of the challenge is to sink the ball in as few shots as possible. The portals form a shortcut to the green.
The way I got this was to aim at the right-hand portal, which should be about 260 yards away over a bunker. Your target is 100 feet lower than the tee, and there is a 10MPH wind, so be sure to account for that in your aim, or use the Arcade game type, as it will allow you to zoom in on your target and give you Focus Aim.
Switch your club down from a driver to a 3 Hybrid to account for the elevation. Depending on your golfer's stats, you may need to tap the A button a couple of times on the back-swing to add a touch of power in order to reach the target.
Once the ball flies into the portal, some crazy things will happen, your ball will appear on the green, and the achievement will pop. It may take a couple of attempts to get the line and range right, but it shouldn't pose any significant challenge.
