  You'll get this when you fight Worms that explode after you hit them (3-14), if you don't or are stuck at 95% then 2 you are likely to miss and may need to replay are 2-22, Vampire Chicken or you may have snuck past your first encounter with Jesse and Clyde on 2-33.

  I was also missing the haunted chair, but also the spider. I guess it only counts the first time you see them since all the subsequent times I encountered them on my playthrough didn't count either.

  Also got stuck at 95%, but mine ended up being the Skeleton. I guess running right past them in 1-3 means you didn't "encounter" them.

  Any advice; I have the milestone at 100%, but the achievement is only at 90% :(

  Simply play through the game and you will naturally come across all of the ghouls. IIRC it was the worm that is the last Ghouly

  In my case, the counter for the achievement stayed at 95%, even after finishing the game. I looked this up to see if someone else had had trouble with this, and sure enough, found that someone else had. He said that the game hadn't counted the Vampire Chicken, so I loaded up Chapter 2-22, The Chicken Shed. As soon as I encountered the Vampire Chicken (with its name displayed), I checked the milestones again, and the counter had hit 100%. The achievement popped up soon after.

  Thanks Sir Voltage. I made it past the worm with no cheevo so tried your method and sure enough it was the vampire chicken!
