This achievement is for collecting all of the Cog tags, this includes the 30 standard cog tags from the original and the 3 additional cog tags in the extra levels from PC. I would suggest checking out the video which will get a great visual guide, but will put the text guide below for all of them as well. If you need clarification for any let me know and I’ll try my best to make it more apparent. Guide courtesy of Gamerheadquarters

    Act 1: Ashes
    Chapter: 14 Years after E-Day
Cog Tag 1: When you start the game you can choose a path, regardless of the choice you can head straight ahead towards the giant red Gears logo straight ahead and find the tag underneath.
Cog Tag 2: This tag is located in the first room where a door is cut open by the locust. It’s in the rear of the room under the stairway you came in on if you took the training route. There’s a massive Gears logo on the wall and its underneath.
Cog Tag 3: As soon as you enter the courtyard before running to the King Raven just cut left up the stairs to the side and you’ll find the tag in the first open space. There are enemies around, but it’s quite easy to nab without any conflict from them.
    Chapter: Trial by Fire
Cog Tag 4: This is the most apparent since one of your AI comrades will point it out for you. It’s also got a massive Gears logo around it.
Cog Tag 5: After you encounter an in-door area with two emergence holes you can find it to the left of the exit beside some weeds.
    Chapter: Fish in a Barrel
Cog Tag 6: After some combat with trokas in the open street you will come up to a massive fountain, head to the left of the fountain and find the cog tag against the wall. This is quite visible in the opening for collection.
    Chapter: Knock Knock
Cog Tag 7: Before battle in this large and open area you will see a van to the left with a Gear logo on it which is hard to miss. The cog tag is just to the side of it.
    Chapter: Hammer
Cog Tag 8: This is found in the wretch hallway at the far end beside a cabinet. Try to pick this up before or during combat to make sure the cinematic activates and gets in your way.
Cog Tag 9: This is found in the room where Rojas is killed or if not paying attention to the audio a body in the middle of where you need to head, look to the right of the body and there it is.
    Chapter: China Town
Cog Tag 10: After seeing a cog get mowed down and beaten by the berserker during a cutscene just turn around and head to where his body is, cog tag is right there.
Cog Tag 11: After drawing the berserker across a number of rooms, at the third room you can find the cog tag at the bar back to the left. It’s nestled in the corner and easy to spot.
Cog Tag 12: Once you have drawn the berserker outside head to the far back left and the cog tag will be tucked away in a corner behind one of the blocks sticking out from the wall.
    Act 2: Nightfall
    Chapter: Tick Tick Boom
Cog Tag 13: Right after re-grouping you will come to an area with metal stairs, head to the right and go down. You will see a big couch and some ammo, the cog tag is right beside the couch.
    Chapter: Grist
Cog Tag 14: Once you exit the square shaped catwalk room descend down the stairs and go the back of the alley stairs you were on. It’s just nestled in that area and should be mostly visible.
Cog Tag 15: Just before the stranded settlement you need to head left behind some newspaper areas and the cog tag is located right there, make sure to get before the cutscene.
    Chapter: Lethal Dusk
Cog Tag 16: On the way to checkpoint 2 the road curves off to the right. Before heading that way look to the left and shoot a gas canister in a shed, the cog tag is inside. Be cautious of getting attacked by the Kryll.
Cog Tag 17: This is another easy one and located at the section where you guide Dom with the flashlight. This is easily down in coop in guiding Dom over there or you can grab the tag after Dom lights up the street.
    Chapter: Dark Labyrinth
Cog Tag 18: This is located where you need to shoot a gas canister inside a car after exiting a building. Just look to the right after leaving the building and it’s right there.
Act 3: Belly of the Beast
    Chapter: Downpour
Cog Tag 19: On your way around the generators you will see a building with a 3 on it or see a long platform heading out to sea. The cog tag is at the end of it, worry not about the water since it doesn’t affect you.
    Chapter: Evolution
Cog Tag 20: This is found at the back right of the rotten floor boards section. Be cautious not to fall through the floor, this area will also directly take you to the exit after.
Cog Tag 21: Downstairs in the cart control room to the right of the stairs in the corner. Be sure to collect it before pressing the button or entering the carts, its right beside the carts if you’re having trouble.
    Chapter: Darkest Before Dawn
Cog Tag 22: After riding down the drill platforms you can find this tag located on the right side after advancing a bit.
Cog Tag 23: When in a battle after contiueing along you will see a platform section with a bit of ramp on the right side. Just to the lower side of that ramp will be the cog tag.
Cog Tag 24: This is right before you take on Corpser to the right beside some rocks. If you see a wide open area with a pool of emulsion around it and the place looks like a boss fight area you’ve gone too far.
Act 4: The Long Road Home
    Chapter: Campus Grinder
Cog Tag 25: Head straight from the Raven drop-off. It’ll be to the right beside from stairs, when looking outward the whole courtyard should be visible.
    Chapter: Bad to Worse
Cog Tag 26: Right after the cutscene with some wounded Gears being searched you can head to the right and find the cog tag in the open.
    Chapter: Imaginary Place
Cog Tag 27: When in the Fenix Estate after leaving the lab section you will be ambushed by some wretches. Shoot the desk, the cog tag resides within.
    Act 5: Desperation
Chapter: Comedy of Errors (New PC)
Cog Tag 28: When you exit one of the later gardens to your left will be a partly opened gate. Head behind that area you find the Cog tag.
    Chapter: Windows Shopping (New PC)
Cog Tag 29: As you exit the underground parking lot you will need to head up by going the left, if you head to the right down lower you will find the cog tag on a stair.
    Chapter: Jurassic Proportions (New PC)
Cog Tag 30: When fighting the Brumak head to the far end of where you start, the cog tag will be behind barriers in the middle of the back against rubble.
    Chapter: Train Wreck
Cog Tag 31: This is in the first passenger car you enter, right to the left just after Jack rips the door
Cog Tag 32: In the third passenger car to the right just before you head into the room where you can head up a ladder. Don’t go up the ladder until you have that cog tag.
Cog Tag 33: This is in the fifth passenger car in the cabin area. Once you release the cylinders head through that area and turn left to the open area opposite the cylinders. You will need to open a door with a green indicator. Hope this guide helped you.

Scream26This guide helped me get my collection achievements and I prefer text to video in scenarios like this, but the descriptions could use a little work. I had to look at the video a few times for clarification and even then, some of the guide wasn't as detailed as I would've liked. For example, for number 23, the text guide is vague as there are "bits of ramp" throughout the level and, in the video, you said you couldn't remember if it was located before or after the waterfall section. That's a bit frustrating because there's a checkpoint after the waterfall, so someone would need to restart the level to find the tag if it was before the fall (luckily, it isn't). A few more details would greatly improve the guide, but I'm still giving a thumbs up because you helped me get the achievement.

The game has total of 33 collectibles in 5 Acts. All collectibles are immediately saved if you pick up them. In the main menu under the point COG tags / comics you can see how many collectibles you have in a act, each comic is one act. None of the COG tags are missable, you can replay every time you want a chapter.

I have one video for all 33 collectibles. The video shows in progress of the mainstory the collectibles, you see also the current missionobjective, the act and the COG Tag count in the left upper corner of the video.
