  If anyone is having trouble with the switch puzzle towards the end of the chapter...
  
  EDIT: A few commentors have stated that my solution was different to theirs. If this does not work for you, I am sorry, as it does seem to be a bit random.
  
  EDIT 2: A common solution in the comments is switches 1, 3 & 5 UP, with 2 & 4 DOWN. I hope that this helps.

  1, 5, 3 up and 2 and 4 down in that order worked for me

  The achievement won't come through even after I rewind to chapter 5 to do it again
