The reason that the achievements go from Chapter 1 to Chapter 4 is because the achievements where you go put and acquire one of the members of the OOPS count as chapters 2 and 3.

In chapter 4 you start off heading back to the temple, when you get to the temple you need to activate the tracking system by pulling the lever again, you also need to a talk to everybody. This time 2 new colors will be on the map, one is Siorn and one is Ivor. You need to get to Siorn before Ivor does. You can then choose whether or not to start right now or start in the morning. The achievement should pop after another weird cutscene.
