Go to your options tab -> matchmaking preferences and select 'TDM- Hostage' as the only active gamemode (switch all the other gamemodes to 'off')

After that switch all maps to 'off' except for Hereford.

According to the game you should now exclusively be confronted with Hostage scenario's (even though i personally occasionally still got matched in a defuse bomb scenario) and have a higher chance of getting the Hereford map. (map rotation is based on the preferences of ALL players in the lobby so there is no way you can guarantee that map, only increase your odds).

Unfortunately the way this game works is that if all others players have all maps set to 'on' then it will simply ignore the fact that you have switched all other maps to off because you have been outvoted. The only possible course of action is to simply keep playing or and hoping that eventually Hereford makes an appearance. Alternatively you can just keep searching lobbies until you find it eventually.

All that remains is being matched with teammates who are actually capable of winning the game.
