  This steam guide here https://steamcommunity.com/sharedfiles/filedetails/?id=38288... has a nice map linked below with the bombs. You need to pick up the 5 with red fuses WITHOUT PICKING UP ANY GREEN ONES. The map is just a reference though, you can see the color of the fuses if you look hard enough.
  
  I accidentally picked one up, I simply dashboarded and got all the red ones and it still counted. I'm not sure how often the game autosaves and in what situations it does, so this may not work if you wait long enough, or due to other circumstances. Best bet is checking the fuse color.

  Achievement didn't pop for me after turning in the quest. I then checked the in-game achievements page, when pressing the "back" button and go to the achievement page with "RT" or "LT". It unlocked on that page (was in color), so I pressed the xbox button to get to home screen and as soon as I went to the achievement section, the achievement popped. Was stuck somehow....

  This achievement is bugged for me, I have restarted the game 3 times already and correctly taken 5 red bombs and turned in the quest. The achievement is unlocked within the game ui but not for my Xbox live account. Idk what to do just felt like venting
  
  EDIT: I was able to get the achievement by deleting my account data for the game off the cloud and starting entirely fresh.

  This achievement is "missable" and the quests starts in Markovna by Dragan. In Gallowsbog in the red marked area on my map below are ten rusty bombs.
  

  

  
  Five of them have a green fuse and five bombs have a red fuse. The bombs with the red fuses are the right bombs. Don't touch the bombs with the green fuse, otherwise the achievement is gone for this playthrough and you have to start a new playthrough or create a new hunter and play again to the quest.
  
  After you have collect the five red fused bombs, go back to Dragan in Markovna and earn your achievement.
