How to unlock the Booked Em achievement

  As far as I know you must use the Cure-All on the "unicorn" to get this achievement.
  
  After returning the "unicorn" to the merchant you will get an axe. Use that axe on the tall mushroom where the page on the prison level rests.
  
  The "heart" page is in the locked room with the flour.
  
  The "bravery" page will go into your inventory when reading some pages posted on a wall near the pipe you enter to get the bolt cutters. In the area after you use the bomb to blow up the fireplace by the "Cinderella scene."

  I believe it is your normal XBL issue during high volume times of the year. The cloud based achievement system always has issues when tons of gamers are ulocking achievements.
  
  I know other people having similar issues in other games.

  I got a glitch where the bomb didn't appear in my inventory rendering destroying fireplace fucking impossible

  I know this is a year late, but if you went to bed with the bomb in your inventory the goblins take it and it's put in a small cage next to where you get the chundershroom. You need the chopsticks to pick the lock and retrieve your contraband items.

  On a side note I recommend at the beginning of DAY 2 to go to the main menu and copy your save game to another file this was you wont need to play the whole episode again to mop up remaining achievements
