Progress through the chapter as normal, up until you purchase the cure-all. Instead of giving it to the baker's wife, give it to Mr. Fancycakes. He will then join your inventory. Go back to the shopkeep and give him Mr. Fancycakes. He'll give you a small ax in return. The achievement will pop as you can start moving again.
