  Reach the inner circle, this means that you have to earn the Rank of a Disciple as a follower of the Faceless.

  With this new DLC, the Following, you have to earn the trust of a religion/group(?) called 'the Faceless'. As you progress and complete missions you will earn x amount of trust exp to rank up the level of trust.
  
  There are 6 levels of trust and with each rank you get a reward;
  - Outsider (no rewards, you start as an outsider)
  - Stranger (Stranger Outfit)
  - Newcomer (Compound Crossbow, Regular Bolt)
  - Ally (Ally Outfit, Elemental Bolts)
  - Believer (Upgraded Crossbow)
  - Disciple (Acolyte Outfit + Achievement 'I was waiting for you for so long')

  
  
  How to get Exp;
  You get experience by completing story quests, side quests and survivors who need help in the countryside. There is actually no need to farm these survivors missions because you rank up pretty fast therefor you will definitely earn this achievement along the way!

  Killing bolters and returning their livers to Jasirs farm is another way to get points toward the Disciple rank. Unlike in the base game, the bolters appear during the day - I found the best way to take them out was to sneak up on them and kill them with the bow/crossbow.

  Also note, you can duplicate bolter liver to get your 250 legend in the process.

  I had a few stacks of 99 disaster relief packages saved over from getting legend rank 250 in the base game. I turned in a couple of those stacks and hit disciple rank instantly.
