This is not intended to be a full guide for the achievement, but a fix for a known bug that I got hit with and found the solution for online. This bug seems to affect all systems (ie PC, Switch too).

If, after you have been to every area and collected every item, you are stuck at 99% in Moon Grotto, you may have been affected by this bug. There is an area at the bottom of Moon Grotto that is covered by lasers (below where my Ori is on the map pic included below). You are required to go through this area in order to progress the story, so it should be cleared, BUT, if you died after completing this area, but before saving, it seems to trigger a bug where the lasers are on, but the area is not considered cleared. There is no second block that spawns on your return visit to this area to block the lower laser. IF this happens to you, you can use the following cheat code to jump over to the handle that turns off the lasers:

cheat code: up, right, down, left, right stick press, up, up, jump

To do this, just hang on the wall above the bottom laser, and enter the code quickly with your controller. You will teleport over and above the switch, allowing you to traverse the area below a second time and get credit for the full map clear.

I can personally confirm that it works on the Xbox One and it saved me several hours of another replay. The second screenshot is of me on the "correct" side of the laser after entering the code. I hope this helps you too! If you're leaving negative feedback, please comment as to why, so I can improve this solution.

Armstrong x360aOof. Yea, only works if that’s the part you’re missing. There are tons of tiny hidden spots. Usually they have an energy globe thingy (forget what they’re called, been awhile since I played), but not always. There is a thread in the forum here of all the spots people had missed. At least you have it narrowed down to Moon Grotto, right?

UGWRayneNo i havent 100 percent narrowed it down because im using the 3 page walkthrough on this site and im 3/4 done with the 2nd page.

Armstrong x360aHmm, you’re right, I can’t seem to find the thread I was thinking of, might be under a different version of the game or on xboxaxhievements…one trick might be to find a pic of the completed Moon Grotto and compare it to your map looking for any shaded areas.

PrimeBigTimeIs this missable because you can't go back to certain areas at the end of the game or is it technically not missable because you can reload your last save and go back to everything?

hanno67It is very likely missable because of Misty Woods, as the area changes every time you'll find a key stone, which will cut you off from any areas you have not fully explored later on.

Armstrong x360aI think the missable tag is appropriate.

X2poet100% maps do not include secret areas that have no collectables.
I freely cleared easy mode, then 100% the maps without any trouble. So maybe it is not missable anyway. But my "find all secrets" achievement did not unlock. So I will search all the hidden area again to confirm.
If you want to research Hidden Area in Misty woods, you can pick up the spirit ball to cancel the clearance of the mist.
    I'm in NS platform first.
