Just putting it out there right now... this is hard. (That's most likely why no one has it yet.) There aren't any shortcuts to this.

Hard is especially difficult because the game doesn't pause while making decisions during combat. You will need to know what you are about to do quickly. That requires a fair amount of familiarity with the game and the combat system. Since you need to make different decisions in different playthroughs to unlock all of the Codex entries, I suggest doing this as a separate playthrough.

The main points are to play cautiously. He suggests using a Mace as your starting weapon, and choosing Healing, Mindblast, Weaponskill, and Sixth Sense.

Mindblast/Mindshield is essential in Vordak fights, since it usually starts with a psychic attack. It is also a strong attack on its own. It does not work against some enemies.
Healing will complete refill your health and endurance in combat.
Weaponskill prevents you from using any other Kai abilities in that turn, but allows you to attack without using endurance.
Sixth Sense helps with the cube puzzles, and helps defend when your attack is finished.

Start every attack with the Sommerswerd, if possible, although sometimes if you know that a stronger enemy will show up as a reinforcement, you can hold onto it. Otherwise, it can kill one or two weaker enemies in your first attack (or at least weaken them).

Try to end every turn with either your defense move (B and bottom left) or Sixth Sense.

Later in the game, I found it better to use two weapons. Attacks with chains are good because it prevents enemies from parrying and riposting.

Important tips: make sure your potions are full and ready. Upgrade your armor and weapons whenever possible. Farm enemies by traveling the map and being ambushed in order to find materials and gold for upgrades.
