This is given for defeating two successive boss fights against three lineage types at the top of the Mausoleum of Storms.

The first fight is the more difficult one, as the ghost ship deals massive physical damage. I put up a strategy in another achievement for a later boss that should probably work here as well, but I didn't have the party setup at this point, so I haven't tried it myself. The idea is to have a character that has been a high level mage and cleric spam 'Multi-Avoid' with 'Master Cast' every turn to the point where no physical attacks will hit them, while a cleric casts divine armor on each party member, and everyone else defends as much as possible. If you too are unable to do that at this point, I managed to beat this boss with a mostly turtling strategy.
Using the following every turn (minus using Holy Weapon from the cleric on the first turn to be able to hit the boss.): Black Wall (Divinity), Illusion (Ninja) [I had two ninjas doing this, no idea if it actually stacks or not though.], Divine Armor (Cleric) [Once per party member, weakest ones first], Iron Defense (Knight). Have the Cleric heal if absolutely necessary, anyone else (In my case it was a mage and ranger) should be doing as much damage as they can, thankfully the boss doesn't actually have a lot of life and I was able to take it out with just the mage using two firebolts a turn and the ranger using random shots each turn.
The second fight is actually much MUCH easier as neither Charon or the ghosts he summons do anywhere near as much damage as the ship, and should be killable without any particular outstanding strategy. The only thing I noticed to lookout for this fight was Charon lowering the party's morale right before Ohma's Impact, so be sure to have Sword Bond up early.
After defeating Charon you'll get the achievement, but be careful on your way out of the dungeon, as the traps (which never triggered for me before beating Charon), can hit you and kill your party fairly quickly if you run over too many of them too fast.
