There are 35 Miracles in the game:
16 that can be bought from NPCs
11 that are found in static locations
5 that are made by transposing boss souls with Ludleth of Courland in Firelink Shrine
3 that are covenant rewards (Sacred Oath, Great Lightning Spear, and Darkmoon Blade)
This achievement can be earned in your first playthrough or across multiple playthroughs. There are missable Miracles tied to
Irina of Carim,
Yoel and
Yuria of Londor,
Ringfinger Leonhard and
Siegward of Catarina, so be careful with their progression.

Great HealFrom the Hidden Manor bonfire, backtrack out into the watery area and stick to the left side. The miracle is on a dead creature at the base of the bridge where you entered the area.
Soothing SunlightTrade with Ludleth of Courland for the Soul of the Dancer.

Bountiful LightSold by Irina of Carim after returning the Braille Divine Tome of Lothric.

Seek GuidanceOn a corpse in the cathedral of the deep. After running around the ledge where the giant tries to smash you, go down the hall, take the first left, and you'll see the item on a corpse on the other side of the room. Beware the slime above the corpse.
Sacred OathEarned by reaching rank 1 in Warriors of Sunlight (requires 10 Sunlight Medals). You earn a Sunlight Medal for every successful co-op session either as or with a Warrior of Sunlight. The knights in Lothric Castle have a small chance of dropping them, too.
ForceSold by Irina of Carim after returning the Braille Divine Tome of Carim.
Emit ForceGiven by Siegward of Catarina in Ithiryll of the Boreal Valley as part of
his questline.
Wrath of the GodsFrom the Profaned Capital bonfire, proceed to the toxic swamp area, then climb the ladder and work your way onto the roof. You can drop down into the building to land on a platform where this miracle is on a corpse.
Lightning SpearFrom the Old Wolf of Farron bonfire, take the lift up to the area with a Stray Demon but go right and drop to a ledge that lets you get to the area behind the huge portcullis. The miracle is on a corpse in this area.
Great Lightning SpearEarned by reaching rank 2 in Warriors of Sunlight (requires 30 Sunlight Medals). You earn a Sunlight Medal for every successful co-op session either as or with a Warrior of Sunlight. The knights in Lothric Castle have a small chance of dropping them, too.
Sunlight SpearTrade with Ludleth of Courland for the Soul of the Lords.
Lightning StakeObtained by killing the giant worm in Smouldering Lake near the Demon Ruins bonfire.
Lightning StormTrade with Ludleth of Courland for the Soul of the Nameless King.
Divine Pillars of LightTake the lift from the Grand Archives bonfire, then go outside and take the steps on the left to circle around onto the roof. Proceed until you reach a point where you can drop into the building. You'll land on a cage of sorts where the miracle is on a corpse.
Blessed WeaponSold by Irina of Carim after returning the Braille Divine Tome of Lothric.
Lightning BladeFrom the Ithiryll Dungeon bonfire, go across to the opposite side of the large area, drop down a level, and go through a shortcut door that leads to a lift. Jump off the lift halfway down and the miracle is on a corpse.
Darkmoon BladeReward for reaching rank 2 in the Blade of the Darkmoon covenant. This requires 30 Proof of a Concord Kept, which are awarded for successful Blade of the Darkmoon invader kills. These "invasions" happen automatically when you are in the covenant if a member of the Way of Blue is invaded- you are summoned automatically to protect the host. It doesn't happen very often :(
They are also very rare drops by the Silver Knights in Anor Londo. This was my farming technique:
You can find 2 Proof of a Concord Kept per playthrough. You'll have to farm the rest.
Magic BarrierSold by Irina of Carim after returning the Braille Divine Tome of Lothric.
Great Magic BarrierFrom the Great Belfry bonfire in Archdragon Peak, proceed outside and the around to the right to find a ladder that leads up to an area with a dead dragon and Havel the Rock. On the left side is a break in the railing that you can go through to drop to a ledge where this miracle is on a corpse.
Dark BladeSold by Karla after returning the Londor Braille Divine Tome.
Vow of SilenceSold by Karla after returning the Londor Braille Divine Tome.
Dead AgainSold by Karla after returning the Londor Braille Divine Tome.
AtonementFrom the Farron Keep Perimeter bonfire, head away from the Abyss Waychers area and drop through a hole in the wall on the right to a ledge where the miracle is on a corpse.
Deep ProtectionSold by Karla after returning the Deep Braile Divine Tome.
GnawSold by Karla after returning the Deep Braile Divine Tome.
Dorhy's GnawingFrom the Centeal Irithyll bonfire, proceed until you see a circular platform on the left with a crystal lizard. There is an illusory wall (a railing, really) that you can attack to open a path. You can eventually drop into an area behind a locked door with an enemy that drops the spell when killed.
Lifehunt ScytheTrade with Ludleth of Courland for the Soul of Aldritch.

Relevant NPCsIrina of CarimRequires the Grave Key from the Handmaid in Firelink shrine, which requires returning the Mortician's Ashes found in the Undead Settlement graveyard area. You can then unlock a door in the sewer area to the right of the Dilapidated Bridge bonfire in the Undead Settlement that Leads to Irina. Exhaust her dialogue and she will relocate to Firelink Shrine.
KarlaLocked in a cell on the lower level of the Irithyll Dungeon. The key is found in the Profaned Capital at the end of a hallway in a building beyond the toxic swamp. Exhaust her dialogue and she will relocate to Firelink Shrine.
NOTE: Karla will accept two of the Miracle tomes (Londor Braille Divine Tome and Deep Braille Divine Tome) after an initial refusal. Irina will accept these tomes, too, but will be driven mad by them and leave Firelink Shrine shortly thereafter.

Braille Divine Tome of CarimBacktrack from the Halfway Keep bonfire in Crucifixion Woods to a bridge. You can drop to a ledge below and the tome is on a corpse nearby.
Londor Braille Divine Tome Sold by Yuria in Firelink Shrine.
NOTE: This tome is missable as Yuria will not appear unless you level up five times through Yoel of Londor before defeating the Abyss Watchers and entering the Catacombs of Carthus.
Deep Braille Divine TomeDropped by a mimic in the Cathedral of the Deep in a room close to the large area with the two giants.
Braille Divine Tome of LothricBacktrack from the Dragonslayer Armor bonfire and look for a break in the railing on the right side. Fall down and the tome is on a corpse in the room.
