  This achievement requires you to assassinate Silvio Caruso in 3 unique ways; by killing him during a therapy session as his Doctor, by shooting his plane with a cannon as he escapes and by sniping him through the lens of a telescope as he looks through it. These assassinations can be quite tough to do, so I've assembled some walkthroughs on the best ways to do each one as quickly as possible without any crazy high level equipment or spawns.
  
  Trinity #1 - Therapy
  

  This requires you to dress as Dr. Lafayette who is Caruso's psychiatrist. From the main square, find him on the phone on a balcony near an ice cream shop and listen to his phone call. Now, follow the restaurant waiter on his smoke break, take him out and take his disguise. As the waiter, climb into the window of the cafe (on the main floor) and steal the rat poison just inside the window. Go back and serve the doctor the rat poison in his drink. Follow the sick doctor to the washroom where you can choke him out, steal his disguise and stash the body. Now enter the compound as the Doctor (without a weapon), talk to the butler, go to the private room, wait for Caruso, and assassinate him during the session via fiber wire or smothering on the couch.
  
  Trinity #2 - Plane
  

  This is a little in-depth, but isn't bad if you pay attention to detail. Go to the church and follow the lab tech from the confessional, to the alter, to the candles and wait for her to leave the church. Enter the morgue from the front, take out the clerk, steal his key, stash his body and wait for the lab tech to be alone in the next room. Go into the room, take her out, steal her biolab access card and laptop dongle. Next steal the lab tech disguise off the dead body on the table. Now head along the shore to find the hidden door embedded in the rocks that enters into the ether lab area. Head through it, and up the stairs to end up inside the compound nearest the telescope room. Watch out for enemies, and pull the fire alarm just outside the room once Caruso enters. Quickly head back into the ether lab, downstairs, and use the stairs to get on top of the bunker with the laptop and lab tech. Wait for Caruso to enter the cavern for target lockdown. Use the laptop (do not take out the lab tech), to kill the virus and start another high level alarm. The target will go into lockdown again, and he will try to escape. If he doesn't try to escape, you either used the laptop too early, too late, or were spotted as a suspicious person. Once you see the "target escaping" message, run towards the plane, out the cave opening towards the ruins. At the ruins get on the cannon closest to where you enter from with gun powder and a cannonball on a barrel. Load the cannon. Wait for the plane to cross in front of the lighthouse, and fire the cannon. There should be 0:10 left on the escape timer when you fire. It will take out the plane.
  
  Trinity #3 - Telescope
  

  This requires a Jaeger 7 (Sniper Rifle) to be spawned as an agency pickup. I recommend the ICA Safe House for the dropoff location as well as the spawn location. Spawn in, pick up the sniper, use the rooftops and ledge to sneak into the kitchen, through the kitchen and out using the window. Turn right out the kitchen window and head into the open window in front of you into one of the main buildings of the compound. Once inside, go through the washroom, and up the staircase. Once at the top of the stairs, find the telescope and video room nearby, and access the telescope controls to open the dome. Head back to the ICA Safe House using the same route after doing this. From the Safe House, go to street level and sprint towards the church. Access the church using the scale nearest the pier to avoid as many enemies as possible. Running around with a giant sniper rifle on your back will get some unwanted attention! Access the church through the back entrance, and go up to the stairs to the top of the bell tower. If you're quick, you'll lose all the enemies along the way, otherwise 1-5 of them may follow you up there, which you can easily take out with the pistol. Otherwise, from the top of the bell tower, aim towards the telescope dome to see the opening. (I had a graphical glitch, but) you should see the round of the telescope poking through the dome, with a blinking eye inside. Snipe this and you will complete the assassination. If you get the same glitch as me, just sniper the blank texture in the right place to trigger it as seen in the video.

  For #2, I just started shooting everybody. You may have to kill a bodyguard (not sure exactly what does it), but it’ll trigger his escape. The only downside is that you will be hunted, which means you can’t save. But I was able to shoot down the plane on my first try. Way fewer steps this way.

  Great guide, worked first time for me, however the tracker stuck on 33% but the achievement popped after sniping the guy.

  You don't need to bring a sniper rifle to the telescope challenge, I just did it with the regular silent pistol. You can shoot the power box and the telescope from the top of the church tower just fine with the silencer.

  Here is my video and strategies for this achievement.
  

  The Basic steps which I used in my strategy:
  00:09 - Therapy Kill
  
  

  *** Spoiler - click to reveal ***1) go to the Cafe back door and subdue the kitchen assistant
  2) disguise and pick up the Town Hall Key)
  3) open the door with the key and put the body inside in a dumpster
  4) go upstairs and collect the rat poison
  5) go outside and use the rat poison with the glass on the table
  6) outside go closer to the therapy guy and activate the opportunity with the doctor
  7) now wait and follow the doctor to the toilet, subdue him and disguise yourself as doctor
  8) put your weapon on the ground! (important)
  9) now go the mansion maingate then upstairs and follow the butler and wait
  10) kill Silvio

  
  06:26 - Cannon Kill
  
  

  *** Spoiler - click to reveal ***1) starting point "Undercover in Mansion Kitchen" if you want to save 1-2 minutes
  2) from the kitchen cross the courtyard and jump into the window
  3) go upstairs to the observatory and pick up the VHS Tape
  4) go downstairs in the media room, put the tape inside the player and pick up from the table the biolab keycard
  5) if Silvio going in direction to the media room, activate the fire alarm and run as fast as you can downstairs and enter the biolab
  6) in the biolab run straight and turn right, subdue the guard but don't hide the body
  7) go in cover and wait that Silvio comes down and discover the body, the body triggers the next step, the target escaping via plane
  8) disguise as guard and run out the cave to the cannon in the ruin
  9) load the cannon and wait that the plane cross the beacon tower (about 11-9seconds left on the timer) and shoot

  
  11:28 - Telescope Kill - Video strategy
  
  

  *** Spoiler - click to reveal ***1) We need a Sniper rifle for this step, so smuggle a Sniper Rifle to the safehouse location
  2) from the original starting point head right side in the small alley (in the near is the back door from the cafe therapy kill)
  3) disctract the guard with a coin, subdue him and disguise yourself
  4) go to the safe house and then inside the mansion, cross the kitchen and courtyard
  5) jump inside the window and go upstairs in the observatory room
  6) activate the observatory and go complete back to the safe house
  7) pick up the sniper rifle and go to the church
  8) inside the church you can go upstairs to the bell
  9) at the top use your sniper rifle and shoot the "eye"

  
  Telescope Kill shortest way
  
  

  *** Spoiler - click to reveal ***1) same steps (1-3) from the other strategy
  2) go to the safe house and pick up the sniper rifle
  3) go to the church and go upstairs to the bell
  4) at the top use your sniper rifle and shoot left side the "power box"
  5) the observatory opens, now wait some minutes until silvio use the telescope
  6) after Silvio used the scope, use your sniper rifle and shoot the "eye"

  you can press the fire alarm at the back of the lab (just behind the two guards) and evacuation will take place. In the mean time, you can disguise in the helmut suit, go in and destroy the virus and you are never spotted. This is the fastest way to destroy virus in my opinion, all you need is the lab disguise and biolab access card. So if you start from the lab, it hardly takes 2 minutes.

  Sure would be nice if you elaborated more on where the back of the lab is and which two guards you're talking about. Been running everywhere in this lab and can't find a fire alarm.

  FWIW on the telescope kill, you can start in the safe house and pick up the sniper rifle straight away. Make sure you have the lock pick as well when you plan the mission. Go out on the roof area as shown in the video but go to the right and drop down. Go though the door and turn right to find a mansion security guard uniform. Now the sniper rifle is no longer an illegal item. You can't go through the main entrance to the church though since a bodyguard will notice you. Go down the steps on the left as you approach the main entrance and you'll eventually see a door on the right. Use the lock pick when no one's around and you can make your way to the top of the bell tower without the body guard (or anyone else) noticing you.

  There are three unique ways that you need to kill Silvio Caruso for the The Sapienza Trinity achievement.
  
  1. Down his plane with a cannon (Error Plane challenge).
  2. During Therapy.
  3. Shoot him through the telescope.
  
  This will require three separate play through's.
