Unmissable story achievement. During Act III, at the very end of the game:
*** Spoiler - click to reveal ***You will fight Belaphoss the Mad while Tasyllys drains the Moontear. Eventually, you'll defeat Belaphoss. At this point, you can destroy the Moontear or have Illydia sacrifice her life to purify it. Either way, the achievement will unlock.
Note; Having Illydia purify the Moontear will remove her from your party. If she's a currently active character, you'll now only have three characters for Phase II of the final fight. On the other hand, purifying the Moontear prevents the bug on the end boss where your party members won't do any actions without specific instructions. So pick your poison.
