This achievement is for finishing the Family Ties companion quest for Dalanir. It becomes available upon recruiting Izhkin in the Forlorn Cliffs during Act III, and is started by speaking to Dalanir in the adventurer's camp.

Below are the steps needed to complete the quest:

1.) Have recruited Izhkin, who can be found in the Forlorm Caverns in Tezzarae's encampment (main quest, unmissable).

2.) Talk to Dalanir in the Adventurer's Camp to receive the quest.

3.) Head to Gracklstugh (the area the main quest takes you to after the Forlorn Cliffs/Underdark Decent) and go to the Shattered Spires (in very western portion of the map, across a bridge).

4.) Speak with Valagh at the inn. This unlocked the Belwar Mine in your world travel menu (under the Underdark tab).

5.) Travel to the Belwar Mine via Word Travel point

6.) Solve the first puzzle in the cave:

a.) The purple gem is already in place, you'll need two more gems to complete the puzzle (accessible through a secret door near the puzzle).
i.) The green gem in the chest in the western part of the secret area
ii.) The turquoise gem is in a crystal batch in the southern part of the secret area
d.) Place the green gem two spots to the left of the purple gem. Now place the turquoise gem on spot left of the green gem. This should open the door. (Note: the yellow gem is not needed to complete the puzzle)

7.) The next puzzle will have you add plates to an alter (the places can be found in chests and pillars nearby)
a.) Place the Bregan D'aerthe house symbol on the alter to open the door to continue your companion quest.
b.) Alternatively, you can place any other plate on the alter to open the second door. Entering this, Dalanir will comment that this is an "obvious trap". Pull the lever in the next room to release three Umber Hulks. Upon defeating them, collect the book for the Alteration Observation side quest (hence why you'd want to come here).

8.) Upon using the Bregan D'aerthe house symbol on the altar, walk down the corridor and speak with the agent guarding Talisstra. During this dialogue, you can convince the guard to give you an amulet he took off Talisstra.

9.) Enter the next room and talk to Tallistra. From here, you have two options:

    a.) Kill Talistra
i.) With this option, you'll have to fight Talistra (not too hard), which will end the quest.
ii.) After this, you can head to the Abandoned Sewers (in the basement of the Cutlass Inn in the market district) and talk to the Mysterious Stranger. You'll get a Crescent Fang (knife) as a reward.

    b.) Spare Talistra
i.) Talistra will curse Dalanir, giving him -1 Int, -1 Con
ii.) You'll have to find a priest to cure the curse. Here again you have two options:
ii.a) There is a priest in Luskan, in one of the houses marked as "Residence". This cure gives Dalanir +2 Int, +2 Con (a net gain for Dalanir of +1 Int, +1 Con). This will cost either 8,000 gold or the amulet mentioned above (the amulet is the better choice, as it has no other function in the game other than selling it for cash).
2ii.b) There is a priest in Gracklstugh, in one of the houses marked as "Residence". This cure gives Dalanir +3 Int, +1 Dex (a net gain/loss of +2 Int, +1 Dex, -1 Con). This will cost either 15,000 gold or the amulet mentioned above (again, the amulet is the better choice, as it has no other function in the game other than selling it for cash).

EYEHUNTER 42Solutions to both puzzles are incorrect.

JohnnyInterfnkThe solutions outlined above worked for me twice. There is a possibility that the puzzles may be different for different run-troughs (I have no evidence this is the case), but saying the solutions are incorrect and giving no further feedback is absolutely useless. What was the solution in your run? What was different in your run? I cannot change the solution above in a void of evidence. Next time, be more specific so we can be more helpful.

Facial La FleurFor me it was Purple in its default position then counter-clockwise there was an empty space, Turquoise, the Green.

VivaJerryThe placements do appear to change. For me, it was Purple in default then turquoise two spaces counter-clockwise from purple and then green one space clockwise from purple.

Check the symbols over the door...you are aiming for white lights at all points of the circle. A little playing around with the green and turquoise gem should get it. I didn't have to use the yellow and blue gems.

I'm not sure what's up with my game, but I got a Blue gem instead of a Green. I had to do:
Yellow to the right of Violet
Turquoise 2 to the left of Violet
Blue to the left or Turquoise
    Remove Violet
