I have created a video for this achievement of my successful runthrough

This is by far the trickiest achievement in the game, it requires a great deal of patience and perseverance to master. In memory 4 of the lost archive, after the first few obstacles you will enter the room containing the animus, it will look familiar as it is where Desmond was held captive in the first game. In the middle of the room there is a hole in the floor which you must drop down to enter the animus.

This marks the point from where you must reach the end of the level without dying, any deaths before this point do not count. You are probably better off doing a few dry run throughs to learn the route and become competent in each area, accept the fact that you will die quite a few times, and this is fine because it's how you learn. Once you feel like you roughly know the route and relatively comfortable then attempt your flawless runthrough.

The video I have provided starts a short way into the animus section. This is due to the 5 minute limit on DVR. The first bit not included is fairly easy, my video contains all of the difficult areas and demonstrates the best way of getting through each one. I have provided commentary to try and explain things a little better. I hope it helps people.

Please give me feedback via comments and if you must thumbs down me explain why so I can edit the solution

EDIT: one thing i did different was on the part of the pit with all the moving objects you have to bypass downwards, i used the triangle as a ramp instead of the straight block as you can then just keep placing them and walk up them until you can drop between each segment, even if it's moving down but with the straight piece you can't as you are on an even footing so the ramp makes it easier just in case you are too near the moving objects. thanks to JPG30 for this info

EDIT 2: FROM THE COMMENTS SECTION, ALTERNATIVE METHOD:

If you are like me and just can't be bothered to do this, then I recommend glitching out of the map and building your way up to the last room:

The video doesn't show or say this, but when you set yourself up on the wall that you will use to glitch out of the map you will have to press forward until it pushes you out. It's a little bit tricky, but you are doing it right, when you see yourself moving backwards by actually pressing forwards. When you fall out of the map and you stop falling, you can just build your way up to the last room. Make sure to actually drop on top of it and then just walk arround a little and the achievement will pop.
