This achievement takes a bit of time (nothing compared to the 500k stars though). At first glance, it may seem that there are not enough levels, but there are more than enough. Here is the breakdown.

Each level has 4 difficulties possible (Normal, Hard, Insane, Nightmare). Earning each of the 4 medals unlocks the next difficulty (Nightmare only is possible to unlock after beating the game - Stage 8 once). This brings each level's medals up 16 possible for a total of 144 medals. In other words, you need to reach approximately 70% of the total possible medals.

In general, to finish the 500k stars achievement in any decent timeframe, you need to unlock the higher difficulties anyway, so you should be aiming to finish this. It might seem daunting (especially the kill all enemies and don't get hit), but there are some decent methods to deal with everything and selectively reach the goals).

NOTE/BIG TIP: You DO NOT need to accomplish all 4 medals in the same level (obviously the 70% and 100% would occur in the same run). Therefore, focus you effort on one of the objectives at a time if necessary. Sometimes, killing all enemies can remove your ability to rescue a civilian or dodge an attack (I took a collision hit with enough planes to guarantee I didn't lose a chain at times). Don't worry about doing it.

In terms of how to do the objectives, here are some tips for each.

General - Get all the upgrades. Everything will help you in general.

1) Kill % - 70% should come pretty easily anyway. The real difficulty is in getting all 100%. To accomplish this, consider using the laser or mega bomb. An additional piece would be to look at including a shield or two. Being able to move exactly to a certain spot at times can difficult depending on how many enemies are present and the projectiles they fire. The laser can quickly rip through any enemy especially the ones that take numerous hits.

If you don't want to waste stars on purchases, consider doing a survey of the level first. Figure out what you can kill already without and determine what needs to change to get them. This can be changing up the kill order or adding a power-up to your loadout.

2) Star % - This only pops up in Level 5. The achievement for destroying the towers can give a bit of an idea to help:

It is possible to get all the stars while losing your weapons, but moving around can get quite tight at times. Just keep in mind that you can't purchase the power-ups so you are limited to what you find and your normal attacks.

3) Civilians - If you focus on taking out most enemies (turrets and anything with an actual attack), there should be time to reach each person. The hardest part for this can sometimes being able to see the civilian.

4) No Damage - This one can be rather frustrating. You could be having a great run and then out of nowhere move yourself into a stray bullet or missile. The obvious fix is to have a shield and power through. The other way to be to use a mega bomb (it also clears the screen of projectiles). In general though, you want to not make a lot of extra movements or place yourself in a precarious position. Some enemies won't even be in your way (helicopters and fighters especially).

For the bosses, a shield combined with a laser can make quick work of most of them (almost feels unfair to the bosses at times). Additionally, some of the bosses actually have large blind spots where they can't hit you. You can actually not move or dodge very little while your homing missiles take it out.

Overall, I completed Levels 1 through 4 completely for the first 64 of my 100. From there, Level 5 I was able to get 8 eventually, but couldn't get much further during the Insane run. Level 6 (Helicopter boss), I eventually was able to get to Nightmare and had most of the stars). I did complete Levels 7 and 8 on Normal and eventually got most if not all Hard medals. Level 9 I only hit 2 (70% Kill and Rescue).

As I said, focus on one at a time and this will come eventually. You'll have plenty of time while going after the longer grinds (cards and 500k stars).
