This achievement is story-related and cannot be missed. You need to progress through about 50% of the game and fight both Jack and Marguerite multiple times, killing them both in the process.

After you escape the old house and take down Marguerite, you'll be forced to find the D-Series Arm that is part of the story progression. Afterwards, you'll be prompted to return to the camper, where you'll be tricked by Lucas and will start his twisted game. This will lead you through the main house again to gather two key cards to gain access to Lucas' mad house.

You'll have to make your way through Lucas' madhouse, venturing through a fat man that vomits everywhere and explodes and a rigged cake, which feels a lot like a Saw trap. After escaping Lucas, make sure you grab all your gear back, as well as the crank and D-Series parts, then make your way to the crank mechanism along the right side of the docks to bring up a bridge that you need to cross. Deal with the 5-7 molded that appear after cranking to reach the last safe room before the second half of the game. Load up on any items in the room and make any ammunition you may need to fight a mutated version of Jack. I recommend flame rounds and shotgun shells, plus any enhanced ammo you may have found or created. Once you feel comfortable, save the game and run upstairs. Give the D-Series parts to Zoe and free Mia form her bonds, then interact with Zoe to start the final Jack fight.

Jack is covered in eyeballs, which are his weak points (obviously). Each corner of the room has plenty of resources to aid you in the fight, so make sure you scrounge them up before the fight finishes. If you're on the top half of the area, he'll use his arms to sweep across in your direction and try to slap you around. Blocking is pretty effective here and gives you enough time between swipes to deal damage to his plentiful eyes on his upper half. The flame rounds are especially useful here, as they deal damage over time to his whole body and can burst some eyes that you never even have to deal with. Once his upper eyes are burst, drop down and start damaging his lower eyes. He will make his way down to you, so you kind of have to do a little dance with him to be able to see each of his eyes. After losing half of his eyes, the room will catch on fire, and upon losing all of them he will burrow into the ground. After a few seconds, he'll grab you and put you in a very limited area while he tries to take you out once and for all. You need to shoot his face while he rears back to slap you, while also defending yourself from his very large arms. Dealing enough damage brings Jack down and prompts Zoe to open up the door. Walk towards Zoe to find out that Jack just doesn't quit (I wish I had his stamina) as he grabs you and swings you around. You'll be prompted to use one of the serums on Jack, which will crystalize him and finally kill him for good.

Follow Zoe to the edge of the dock and you'll be presented with a choice. You can cure Zoe, or you can cure Mia. If you are doing a speed run, you want to save Mia. If you are doing a normal run and you haven't cured Zoe yet, then cure Zoe. Both choices are tied to an achievement, and they will take at least two playthroughs to get both. Regardless, cure one of them and enjoy the cutscene that plays out. Once you gain control of Mia after the cutscenes, the achievement will pop. Enjoy!
