The game is completed when Adam boards the aircraft to leave the prison. The achievement will unlock during the cut scene that follows--provided that the conditions have been met. First,
Adam must refuse the Pill on any occasion that the Fixer might offer it to him. Second,
Adam must not use the Altered Biocell at any time after it has been obtained from the stuffed Teddy Bear in the A Cell Block laundry room. Satisfying these conditions means that Adam must accomplish his mission without augments. The below video shows the achievement unlocking along with two others:
