Not overly secret but I figured I'd add a solution for those who are wondering what this achievement is :)

This achievement is called 'A hero once, a hero twice'. To unlock you need to 'complete the Fallen Ghosts operation'. Basically, complete all missions and kill the final leader who's mission appears in the first province of the Fallen Ghosts DLC.

    Boom. Achievement popped. 50g!

CQOzymandiasCompleted all missions and Rebel Ops, and neither this achievement nor "Thorough Supporter" is unlocking for me. At 6/6, 6/6 and 7/7 missions, and all rebel ops completed.
Even went through and reactivated each mission one by one, as I had seen there were some people who had issues getting the base came completion achievement to pop and this was a solution to that, but still nothing for me.

Hoping someone has a solution since I wasn't a fan of this DLC (enemies spotted me far too quickly compared to main game, at times it seemed arbitrarily as they were not even looking in my direction) and have zero desire to re-complete it on a new save.

Also, it wasn't until I finished all this and was trying to determine if I had missed a mission that I realized there were apparently weapon cases, skill points, and bonus medals that I can collect in each province? Never saw any, anywhere. Game shows me having 1/4 weapon cases in the Eastern District, which confuses me since I do not recall ever opening any weapon cases during the DLC. (I don't have all the skills unlocked, but I assumed the DLC had the reverse of what the base came had, not enough skill points to unlock everything as opposed to more skill points than required). Was there something you could do to make the cases/skill points/medals show on the map? I never even saw them while running around completing missions.

CraigyB78I had to replay the last mission again to get this to unlock.

Genius LunaticSame issue with me man. Neither this achievement nor "Thorough Supporter" pops up for me. These are the only two achievements missing for me. Hate these achievement unlock issues.

Genius LunaticI replaced the Merlo mission again and got this secret achievement but still no sign of "Thorough Supporter". Am so lost now.

MadaI am having the same problem -- cannot unlock neither this nor "Thorough Supporter" though I am 100% on Fallen Ghosts.

I cannot figure out how to restart the Merlo mission. The Eastern Province shows 7/7, but there are only six missions displayed to restart. I have tried to replay the other boss missions, but still will not trigger Merlo.

Also, I had finished all the Rebel Ops before starting Merlo the first time. Can anyone else experiencing this glitch confirm they did the same? I wonder if that is part of the problem.

UPDATE: Good news. Both just popped for me. Not sure what was different, but happy!

smartypants1For anyone with achievement issues, here is what I have found to help. First off I highly recommend Xbox Live. It allows Xbox to connect with Ubisoft servers for player stats. Next if you completed the last mission and it doesn't show up, its most likely glitched.

Pause the game, click Quit Fallen Ghosts, and then you should get the achievement when it says "You are now connected to Ubisoft Servers." If that still doesn't work, try restarting your game.

Even if that doesn't work, unfortunately you may have to restart the mission or contact Ubisoft. I am not sure if Ubisoft even has support for this game as they have shut down Ghost Recon Network.

N1nja P1rateCan't get because I can't complete the Tyrants mission. When I go down the stairs there are no rebels in the cells to rescue. All the cells are empty with the doors open. Have tried restarting the mission a dozen times.
