  Not sure if it changes, but the first questions i was asked were:
  Identify the highlighted position - SAFETY
  Identify the Hot Receiver - X
  What is the play formation called - 22
  A short while later more questions:
  What is the formation - Single Back Formation
  Identify the Safety - X
  Even More questions:
  2nd and 1 - Run the Ball
  What to do instead - Throw a pass
  Several QTE sequences where you have to pick Colt or TE:
  1st - Colt
  2nd - Slot WR (Not Colt)
  3rd - Colt
  During final game, there are several more ways to increase IQ:
  Short QTE sequence to throw to non-covered player
  2nd QTE sequence to check down to Forsett. (You can still unlock if you throw to Colt)
  
  Overall pretty Easy, i missed 1 question early and was still able to finish with an A+.

  Have A+ and still waiting on it to pop into the credits...
  
  EDIT: it unlocked when I finished the credits and went to the MUT.

  To go along with the other solution, I want to note that the achievement does not pop right away. In fact, it doesn't pop for quite some time. It doesn't pop until after Julia follows up with Devin for Longshot.
  
  So don't freak out. If you have a high IQ, it'll pop after a while. I had an A+, but my Performance was an A and I got that achievement. I don't know what the cutoff is.
