This achievement is long and annoying, but don't worry - it's actually not very hard at all. Before we get started, there are a few things you need to know about charges:

1) You have a base maximum of 3 for each kind of charge. Each is counted separately, so we start with a maximum of 9 total charges.
2) You can increase your max charges with passives (3 for each charge, spread throughout the tree) as well as a handful of ascendancy skills and unique items.
3) Charges can be seen swirling around your character while active and as buffs beneath your mana bar. Endurance charges are red (strength-based), frenzy are green (dexterity-based), and power are blue (intelligence-based).
4) Charges have a base duration of 10 seconds (seen on the buff). Each charge type has its own timer independent of the other two. Gaining a new charge refreshes the stack, even if you're at the max for that type. If the timer runs out, you lose all charges of that type.
5) Some skills and items consume charges either when they're used or when you reach the maximum for a certain type. They're few and far between, so odds are you won't have to worry about it, but definitely avoid using these if you do have them.
6) There are several ways to gain charges, but for this guide, we'll be using three skills: Enduring Cry for endurance (grants charges based on the number of nearby enemies; unless there are no enemies, it always gives at least 1), Frenzy for frenzy (grants a charge on a successful hit), and Assassin's Mark for power (cursed enemies have a chance to grant a charge when you kill them); alternatively, you can use a wand and Power Siphon (grants power charge on kill). This eliminates the annoying RNG element of Assassin's Mark, and Frenzy still works with a wand. Credit goes to MushaConvoy for this tip.
7) This isn't relevant to the achievement, but in case you're wondering, each endurance charge gives you +4% to all resistances and 4% reduced physical damage taken, each frenzy charge gives you 4% increased attack speed and cast speed and 4% more damage, and each power charge gives you 40% increased critical strike chance.

Now, since we have a base of 9 max charges, if you're already running a build with a bonus of 4 or higher, you can easily get the last one or two from unique items. Specifically, you should be able to get Kaom's Sign (Coral Ring) for +1 max endurance charges and The Aylardex (Agate Amulet) for +1 max power charges. Alternatively, if you're close to passive nodes that grant more charges, you could spec into those and respec those points after you get the achievement (select the passive you want to get rid of and hit Y, then X to accept the changes). If that's not an option for you, don't worry - it's not required at all (although I do highly recommend picking up one of the two as it will save you a lot of time). If you are using an existing character, you can skip to the end of the guide.

Once your passive tree is complete, socket your gems, putting on the intelligence gear you saved if needed. Take the waypoint to The Riverways in Act 2 (The Southern Forest would work, too) and turn on Assassin's Mark if you've linked it to Blasphemy Support. Huge groups of monkeys come down the trees here. Get as many as possible since Assassin's Mark is a 21% chance for a power charge per kill. Spam Enduring Cry until you're maxed out on endurance charges (6 or 7, depending on whether you have Kaom's Sign), then cast Assassin's Mark (unless it's activated by Blasphemy). It only lasts 6 seconds, so make sure it doesn't fall off before you kill your targets. Landing a few hits with Frenzy and kills with Assassin's Mark will max out your frenzy charges easily (4 if you got a unique, 5 if you used 10 skill points instead) and your power charges with a little luck (4 or 5, depending on whether you have The Aylardex). If you're using Power Siphon, you probably shouldn't need to worry about charges falling off, but if your first pack doesn't give you enough, don't forget to refresh!

If you don't max out your power charges immediately, don't worry. There are plenty of monsters and you can use Leap Slam to get across the river if needed. Make sure you refresh your endurance charges and eventually you'll get there.

EDIT: It's probably worth noting that if you build a Duelist for this, you'll path right past Blood Magic. If you still need the Invested in Blood achievement -
