I did not get this achievement on my entire first playthrough for some reason. Basically I played the game trying to turn on most, if not all the lights. Once the game finished, I pressed continue and it put me back in the last chapter. I was in the room with the main door leading outside. All I did was walk into the door, received a diary page. And kept walking into the door until I found them all.
