Update - Nov 21, 2015: In the August 2015 Version Update, a new Key Item was introduced to the game that significantly reduces the amount of time necessary for Service Evaluations. Specifically, this Key Item reduces the time from 120 real world hours to 1 real world hour, effectively reducing the minimum timeframe for this achievement from 95 days to just 19 hours. As such, priority should be given to obtaining this Key Item:

Campaign is a constantly active event tied to the Wings of the Goddess expansion, with public battles between NPC armies occurring in every area of the world hundreds of times per day. This achievement's key item, the Medal of Altana, is awarded when a player passes their 19th consecutive military evaluation. Service Evaluations can be requested once every five real life days (exactly 120 hours), resulting in this achievement requiring a minimum of 95 days to unlock. It is, therefore, the last achievement to become available for earning after a player starts.

Before beginning Campaign players must align themselves with a "past" nation (this has no effect whatsoever on your allegiance in non-Wings of the Goddess portions of the game) by completing the first few story missions for the expansion, and so in the process of unlocking this achievement it's recommended that you also work towards completing:

Hitting specific milestones in the Wings of the Goddess storyline will prevent you from losing your current service award, up to a certain point, which will otherwise happen if you fail to pass a Service Evaluation for a period of roughly 30 days. Even the Medal of Altana will be lost if you have not completed the storyline and allow the full Service Evaluation cycle to lapse. Manually switching allegiance to a different past nation will lower your service award, again including Medal of Altana, by two ranks regardless of story progress. It is therefore recommended that you initially join a nation that you do not want, complete the recruitment (first) mission for the other nation you do not want, and finally complete the recruitment (first) mission of the nation you do want.

Voidwatch warps to the three cities in the past are unlocked after signing up in the present, available once a player reaches level 75, and are the most efficient way of reaching those cities for the first time.

Campaign Battles -

The "meat," as it were, of this solution is actually the most straightforward. Once you're eligible to participate in Campaign Battles, merely type /cmap to bring up the Campaign Map. Search for the nearest colored dot with a sword icon in it, and head into that zone (if you expend 1 A.M.A.N. Voucher at a Records of Eminence NPC obtain 1,000 Allied Notes, an NPC in each past nation will immediately warp you to any zone you've previously visited for a fee).

When in zone check your /cmap's Widescan for the "Campaign Arbiter," register for the battle through them to receive your Allied Tags, and kill all enemies with sword icons next to their name to end the battle. Speak to the Campaign Arbiter again at any time to redeem your Allied Tags for Allied Notes; if the battle has yet to end, you can immediately reenlist for a fresh pair of Allied Tags to earn more points. Each Allied Tags has an Allied Notes cap, which is hidden but based on current service rank (typically 2,000~8,000), so redeeming them is recommended any time you're waiting on a new enemy wave or find yourself without personally engaged enemies.

Service Evaluations are based on a variety of things, but the largest influence is the number of Allied Notes earned through Campaign Battles (A.M.A.N. Voucher exchanges will not count) during the period between them. To unlock the Medal of Altana with the least amount of energy expended, simply earn 10,000~11,000 Allied Notes in defensive battles (colored circles are defensive, gray circles are offensive) an hour or two before you can request your next Service Evaluation. Should, for whatever reason, you not have enough credit to pass simply choose to postpone/continue the evaluation period and participate in another Campaign Battle or two.

My preferred job for this is MNK99/DNC49, which offers ridiculous offensive and defensive might, decent physical evasion, the ability to heal through Chakra and Curing Waltz III, and the ability to remove status ailments with Healing Waltz. A properly geared Monk can easily fight off a full 20+ monster enemy wave by itself. DNC99/WAR49 is another viable choice for solo play; it boasts significantly higher healing ability at the cost of some offensive power and higher evasion at the cost of lower defense. Both are very easy to gear and play.

Vunkerl Inlet [S] is the smallest Campaign Battle zone (if warped in through the NPC), has an easily managed chokepoint via the bridge, and is by far my favorite area for defensive battles.
