After playing a non-lethal and somewhat lethal playthrough (somewhat because I didn't kill everyone, but some people) I noticed something. If you haven't killed anyone, he will survive the drop through the trap door and just be unconscious. If you've killed people then he dies when he falls through the trap door. It's similar to when you release Daud at the beginning, if you haven't killed anyone then Daud just knocks out the remaining Eyeless in the fighting club.
