A few points:
- Can be done over several playthroughs
- All game modes and difficulties count
- And most importantly: You can see all your completed quests under Pause - > History - Adventure Log

The game has a few stages, on which some quests become unavailable until the next playthrough. These are:
1) "Off With Its Head" (Speak with Ser Mercedes at the Encampment and rest to begin quest chain)
2) 1st Wyrm Hunt Quest (Seek out Ser Maximilian and accept a task for the Wyrm Hunt)
3) "Come to Court" (Accept Ser Maximilian's offer to meet the Duke)
4) "Trials and Tribulations" or "Griffin's Bane" (Accept a royal task from Aldous)
5) "Reward and Responsibility" (Accept Aldous' offer, after completing "Honor and Treachery")
6) "The Final Battle" (Take Greatwall's northern exit to the Tainted Mountain)
7) Post Game (Defeat the Dragon and return home)

All in all, don't stress if you missed a few quests on the first playthrough, the game can be completed in 30 minutes if you just do main quests and know how.
