Crouch and approach enemies from behind, undetected. When you get close, a skull icon appears. You can press A to stealth kill them with the knife.
Do that 10 times.
You could get this in chapter 3 if you stealth killed every enemy you saw, but you'll more likely get it somewhere around 4 or 5. You'll almost certainly get it before the end of the game.
