As with most achievements in this game, make sure you save before going to your final needed event/item. In this case, the last fish. If your achievement doesn't pop, restart the game and try it again.

Pay attention to the kind of fish and their size in the pond.

Fish come in three sizes: small, medium and large, but there's a catch!
The longer you fish, the bigger they get.
So, let's say that you want a Flying Fish (smal and night only). Exit the fishing game, draw the moon, and talk to the fisherman to start fishing again with small fish in the pond. Do not draw during the game.

In the spread, fish have their sizes to the right of the name: (S) for small, (L) for large, all others are medium.

I created a tracking list so it would make everything easier for me and decided to share it. Feel free to save a copy.

    You can sort it by
Fish | Daytime | 1st Location | 2nd Location | which one you have | and where to sell it

Version 2022: fixed most (if not all) prices and added the size.

@illdizzog pointed out this tip to complete the guide: Day and Night cycles continue as you fish. Therefore if you are going for a specific day or night fish you can draw a circle or crescent whilst fishing to keep whatever condition you need present. Thanks, illdizzog!

illdizzogGood guide dude, thanks. Useful tip because this achievement sucks: Day and Night cycles continue as you fish. Therefore if your going for a specific day or night fish you can draw a circle or crescent whilst fishing to keep whatever condition you need present.

Also the fish come in sizes, I used this guide as well:

To pop this achievement, you need to complete the Fish Tome
There are a total of 43 different fish in the game, and you need to catch at least one of all of them.

Warning: The achievements in this game are glitchy, so SAVE when you only need one more fish for your Fish Tome, as the achievement may glitch. If it glitches, you will have to go through the entire game again to get to the point where you can catch all the fish again.

There are four fishing spots in the game. The first is temporary, the other three are permanent.

Agata Forest - Young Boy Kokari - He's here only for the two plot-related fishing minigames. You can't fish here otherwise after these have been done.
Sei'An City (Aristocratic Quarter) - Benkei - Bring Benkei the Blinding Snow fishing rod to catch the 'Living Sword'. After you have defeated Blight in the Emperor, he'll be next to the mermaid spring.
    N. Ryoshima Coast - Fisherman - Located on a long fishing pier on the South-West island. Once you bring this Fisherman the Marlin Rod, he can catch the Giant Marlin, allowing Amaterasu to learn the Whirlwind Celestial Brush technique. From then on, Amaterasu can return to the Fisherman's pier to fish for an unlimited amount of time.
Kamui - Young Boy Kokari - He'll be here after leaving Agata Forest. When the Guardian Sapling of Kamui has been revived, Kokari will be near the Dojo.

Note: You can only fish in Agata Forest for a brief period, and that's when you have to do two plot-related fishing minigames. When you've done these, you'll not be able to fish in Agata Forest anymore. However, the boy who you use to fish (Kokari) will move to Kamui, later in the game. There are no fish (other than the plot-related fish) that are only available in Agata Forest.

The fish, and their main locations, and when they spawn, are as follows:
