  You can equip your train with staff to improve the performance of your train. Every train can have up to four staff-persons: a train engineer, a fireman, a train attendant and a security guard. Select one of your trains and press to show the details of this train. Right in the middle of the screen you can see the four slots for the staff (one slot for each possible job). Select a free slot and you will see a list of persons you can hire or use for this position. Select the person you want to hire by pressing . If you do this with all four slots you have a fully staffed train and will get the achievement immediately.
  
  Note: If there are currently no persons you can hire for one of the slots you have to wait some time until new persons apply for this job (you will get a notification on the top left corner of your screen when someone new applies).

  Does it have to be in a certain mode? I have fully staffed trains in both scenario and campaign so far and dont have the achievement, unless I'm missing something?

  Actually it can be done in every mode. I completed this while playing the campaign. But regarding the achievements the game seems to be a little buggy. Sometimes they just don't pop immediately or don't pop at all (for example: the achievement for the long tunnel unlocked a few hours after i built the tunnel...)
  And maybe you can check if there are really four persons in the same train. Because the list does not only show the free employees, but also the ones currently in use. So if you select one person that is used already in another train it leaves the old train and is used in the new train.

  Yeah it must be buggy as I unlocked the achievement while I was in bed apparently although in the achievement list for the game it's still locked
