The Nightmare Fiend is the final boss located on Floor 20 in Normal Mode. He likely appears in Nightmare and Chaotic as well, but I have yet to verify this.

Since this game is so random, I realize that everyone will have a vastly different run than mine. I just wanted to give some general tips and information on the boss in advance as there isn't really much about it on the internet.

It will certainly take some luck to get down to Floor 20 in a good enough state to be able to defeat the boss. You will not make it there on your first try. You will fail numerous times along the way. You will then make it to him and die numerous times, as well. I lost on him about four times until I finally beat him during a run that I admittedly did pretty poorly on. I just so happened to score a couple of lucky critical hits and have 3 items that worked well against him in my inventory.

My playthrough was with Guy and I started with the God of Rock buff. I like to allocate my stat points like this at level up:

Level 2: +Stamina
Level 3: +Dexterity
Level 4: +Stamina
Level 5:+Stamina
Level 6 and on +Strength

Getting the Stamina point in early not only helps for early durability, but also helps to increase your late-game maximum health as it effects the max health increase that you get from picking up hearts. It's important to get your Dexterity up to 4 early, too, so you can equip a ranged weapon for ranged enemies, or an off-hand weapon instead of a shield for increased damage.

Luckily, on a Floor 18 sub-floor I encountered a God of Contract shrine and immediately switched over to him for the +5 to all stats boost that he provides. I happened to make it to Floor 19 with full health (roughly 250), full soul and zero mana.

My equipment was pretty piss-poor. I actually had the started axe until I bought a replacement on Floor 18. Stay away from gear that increases the amount of tiles that you discover. I like stuff that give you health when you uncover tiles as this helps to keep your health maxed before picking up hearts. The Rattle Snake effect is also a favorite, as well as +Armor gear, or gear that counter-attacks based on your armor value.

You can also keep multiple sets of equipment in your inventory and switch them out as you need it. For instance, keep some high-armor value gear even if you have stuff with better effects. Put the armor gear on right before you go to a new level, then switch it out with the other stuff after you enter. You will still retain the armor value (it only calculates at the beginning of each floor).

I had a Drill (skip a floor), Cross (3 turn immunity to death) and Stunning Hammer (stun a single enemy for 3 turns) as my items going into the boss fight.

Floor 19 is different than other floors. To advance to Floor 20 you have to actually kill every enemy here. At this point the enemies are high level and pretty tough. On my previous runs this had the tendency to wear my character down and leave him in a piss-poor state for the final boss. I used the Drill here, this time, to skip Floor 19 completely and enter Floor 20 fairly fresh.

I then just melee'd the boss until he splits into 4 enemies (at about 70% health.) My melee attack was only 22, and I wasn't dual wielding. I did have 95% hit chance, though. I'm not a fan of the weapon/shield combo, but that's all this run decided to give me. Once he split, I focused on the other four enemies, one-by-one, expecting to die along the way. Surprisingly, only one of them attacks you each turn. After I killed two of them he reformed into one larger enemy. I swear when he reformed he had less health than when he split, but I'm not exactly sure what determines it. It may have been based on the damage that I dealt to his split forms.

At this point I was one hit away from dying so I used my Cross item (immune to death for 3 turns). I also used my Hammer item to stun him for 3 turns because I wasn't sure if he would split again. I killed him before he had a chance.

The God of Rock and God of War bonuses are my favorites to start with so far.

Other items that I find invaluable during runs are the Throwing Knives for ranged enemies, Freezing Crystals, and anything that can kill elites. Petrification stuff can help take out ranged enemies if that is all you have, but you won't be able to achievement the floor-clear bonuses if you petrify stuff.

Edit: As I play more and more and have now completed the game on Chaos and with various Runewords I've started playing in a different style. I prefer to use Morte (or Merlyn prior to unlocking Morte) and the God of Evil - I've written about it in more detail in the Ancient Memories solution. This won't really be relevant unless you've managed to unlock this character and god, though. Using this combination with an Intelligence based two handed staff setup I managed to beat Normal the last 10 times I tried, without fail.
