To unlock a 'Death Route' you need to progress through a level without dying until you reach a 'skull and crossbones' platform, hop on this and the achievement will unlock.

The platform essentially works like the 'Bonus' platforms, and will transport you away to another area with a coloured gem at the end.

The levels with death routes are:

High Time (Purple Gem)
Flaming Passion (Green Gem)
Tomb Wader (Blue Gem)

I reached it on Flaming Passion, by using the bazooka to every enemy (even the ones who throw fire) and taking my time to adjust the jumps, it's not that hard.

Thanks for your posts, gentlemen. Thought, it'd help if the level # was stated for each of the death route stages. For those who'd like to know where exactly to find the mentioned levels:

Level #13: High Time (Purple Gem)

Level #20: Tomb Wader (Blue Gem)

Level #23: Flaming Passion (Green Gem)
