This can be easily achieved in quickplay mode. Set the rounds to 5 minutes and easy difficulty, Build up your combo meter to the second level and perform Groundbreaker using Y. Do this 12 times in a match and the achievement is yours. You can even perform Facebreaker and get the FaceBreaked achievement.
