At some point in the game during a regular level you will see something like a tiny portal or a "tear". Press RB and you will open it and have the option to travel there. It is like any other set of rooms. If you reach the exit successfully you will unlock it (it is not that hard, just totally random the chances to see one)

Challenge rooms spawn in 20% of non boss levels. They are marked by a small rune you must use cn_RB to interact with. Image of the rune below:

Once interacted with, a portal will appear nearby. Enter the portal, take the treasure, and make it to the end if the room. There will be no enemies, but the room will be filled with traps.

Fortunately, using custom seeds does not disable achievements. If you have unlocked the customization rune, then you can run a custom game using seed #804327. This seed has a rift in the opening level ("Prisoners' Quarters") as well as "Promenade of the Condemned," "Prison Depths," and "Ossuary."

To unlock the ability to use custom seeds, you'll need to acquire the rune by killing an elite archer or zombie that is in a room in the "Ramparts," near the beginning of the level. Once you have the option for "Custom Game" on the menu, you can enter a specific seed number in the second customization screen. You can change other settings as well, but be sure not to enable any setting that disables achievements (these are all indicated with a lock next to them).

In seed 804327, the Prisoner's Quarters challenge rift is on the far right side of the map. Just go as far right as you can from the beginning, then go down. It's in a little room all by itself, just a little below the second teleportation portal you find (the portal is just to the right of a treasure chest).
