  Early on in the game you will be shown your rucksack in which you store all the items you find. In there is a hip flask. Do not drink from the flask for the whole game if you want this achievement

  Yeah, It happened to me as well, but again, i was more like speedrunning it, that's why it didn't bothered me that much
