As soon as you spawn (at the default location), go forward and turn right to exit the market, noticing the beggar on the way out. Navigate the alley towards a small plaza with a shoe store. Get inside the store and pick up 5 coins from the counter. Go back to the beggar near the beginning and throw all 5 of your coins at him to unlock this.
