For this Achievement to unlock you simply have to find an eye patch and loot a container with the eye patch equipped. Sounds easy enough, but...

There is no way to tell, where or what apparel item spawns when you enter a world.
Apparel items totally randomly spawn in backpacks and tool boxes (the colorful ones).
You can also find Apparel on bodies.

You really need to be patient with this one, as spawns are totally random.
Take your time and search every house, every car and every body.

Look out for Camping sites, there are usually lots of backpacks around those.
If you have the DLC, a huge one with lots of caravans can be found at Vrakudden Camping Ground, north east on the DLC Island.

Ok, I finally unlocked this ridiculous rng achievement.

My farming route was ...
start : salthamn safe house (2037.221, 3131.710)
search around, go to out of red door.
next : fast travel to Bjorntunet Hotel (4526.050, -68.191)
(You should complete DLC main quests)
search all rooms from 1F to 3F.

If you can't find an eyepatch, delete local save without quit.
Start game again, download a cloud save.
This method saves item respawn time.

I found after more than 30 times attempts.

This took unreasonably long to unlock but finally found a pirate eye patch in klinte. Must've spent 20+ hours on this alone. Was my last achievement to get. +1

The items don’t respawn in the hotel even by deleting local save.

The only place I found eyepatch was South Coast Region. Just follow the shore and loot every bagages and corpses. I found 2 in under an hour. This eyepatch is actually more a disguise, a toy for children. Holidays, families, the shore... That makes sense to me :)

I found one (eye patch, pirate) and am looting items from the world and still no achievement

does it have to be a regular eye patch?

EDIT: I found a regular one 2 hours later and the game didn't add it to my inventory LOL this game I swear

EDIT 2: never mind I think the pirate variant of the eye patch may count, as I loaded up my game tonight and the achievement popped at the main menu. Must've been affected by the issues plaguing some users this weekend.
