All three of these are rewards for completing the "Dragon and the Prince" side mission that comes the DLC, so you cannot unlock this achievement without first unlocking

Once you have completed the side mission, you will need to fast travel to a base camp where you can equip your choice of outfits and weapons. Most of the base camps in the Hidden City, including the Skull Cave, do not allow this. My favorite base camp for purposes such as this is Kuwaq Yaku Ruins in Kuwaq Yaku, but most any base camp outside of the Hidden City should work. Once you're there, equip the Silent Sting - Scorpion pistol and the Hunter's Array - Apex outfit. Note that both of these are variations on similarly named items that were bundled with the DLC, so be sure that you are equipping these exact items, not their more vanilla counterparts. You learned the Raptor's Eye skill at the challenge tomb, so you don't have to do anything at the base camp to make it happen. If you need to check this, press cn_X from the Skills menu at the base camp.

Once you have equipped the pistol and outfit and exit the base camp, the achievement will unlock.

You'll need to complete "The Price of Survival" DLC campaign mission first. Once done, travel to an area where you can wear all costumes, I always head to Kuwaq Yaku village though it doesn't matter.

The "Raptor's Eye Skill" is for completing the mission, the "Silent Sting - Scorpion" is an option in the handguns category of weapons and the "Hunter's Array - Apex" is one of the last outfits. Make sure you wear everything and specifically pick the right outfit, the Achievement will then unlock.
