  While running through the level gilgamesh will stab his legs through the ceiling above and try to hurt you. Just focus fire one of the legs until it backs off and you'll get this achievement.

  In this video guide I will show you how to fend off a menace from above in mission 5. This will be shortly after the mission starts. You will be destroying some of those vines to progress through a tunnel. Once you get to the other side just climb up the boxes to the platform and a tentacle will attack you. Just attack it back and the achievement will pop.

  I'm at the exact same spot as in the video, fended off the leg, and no achievement.
  
  Edit: I got it on my second try after reloading the checkpoint. Go figure...
