For this achievement you will need to firstly find the
Primordial Weapon also known as the
Doormant Handle. When you go back to the Oracle in the spawn area after activating the artifact, she will say
"Seek The Primordial Weapon..."Now there are
20 possible spawn locations of the Primordial Weapon and below I will list all of them with a time stamp so that you don't have to go through the whole video.

Near the wheel of water is in the Cliff Ruins area and it’s on the floor left of the blue fire.

On the broken bridge is in the Cliff Ruins area and it’s on the bridge that you go over and it’s on the ledge.

Chaos of venom is in the Python Pass area and it’s next to the buyable trap just before you go to the area before Pack-a-Punch.

Near the barque of gold is in the Stoa of Athenains area and it’s on a green box.

Where the mighty titan points is in the Python Pass area and it’s inside a crystal that’s on the floor left of the Titan wallbuy.
Now that you've found yourself a Doormant Handle, you need to go to the
Charon Alter that is in the Python Pass area and place the Doormant Handle on it. Then keep on the killing the zombies inside the red circle that appears for a set amount of time. Once that is done pick up the
Fallen Hand of Charon from the alter.
Once you have done that head to the
River of Sorrows which is infront of the Odin perk machine. You will notice a
giant puddle in front of you and you have to kill around 10 - 15 zombies with the
Fallen Hand of Charon to make the puddle go red. You will have enough kills when the game gives you a prompt that says
"drink from the River of Sorrows."Next you to drink from the
River of Sorrows and you need to be on the lookout for coins. Now there are
two types of coins that you can find and they are as follow.
Obtain Charon's Obol (Real Coin)
Extinguish the False Obol (Fake Coin)
Now for this you will need to find
three Real Coins, now the only way to tell if it's a real coin or not is when you go up to it and read the Prompt text the game gives you. You are looking for the prompt text
"Obtain Charon's Obol." Also these coins are really easy to spot as they light up when you have drunken from the River of Sorrows. However, whilst doing this your
health will not regenerate whilst looking for these coins, so I recommend that you do this at the
end of the round with at least 1-2 zombies chasing you.
Once you have found three Real Coins take them back to the altar and a teleporter will open in front of you. Go through the portal, and keep on killing the zombies in the reward area, once that's done will you have obtained the
Redeemed Hand of Charon Gauntlet that is required for this achievement.
Now for the next section you will need to train at least 15 zombies and use the charged attack on the
Redeemed Hand of Charon, you do this by holding down RT and releasing it.
Overall this achievement is pretty doable, I think.
