This solution focuses on an NG+ run. Some tips below will apply for NG. My general advice would be to go one run through first on Easy or Normal, then NG+ on Ultra Nightmare, because to 100% the game achievement wise involves visiting most-to-all areas in the game, and for an Ultra Nightmare run you will want to keep exploration to a minimum to reduce frustration. An Ultra Nightmare NG+ run will take you about 6-10 hours, depending on your general level of luck getting through the main story without dying constantly. Once you get a feel for the difficulty this isn't too hard to clear - you will likely find clearing out bandit dens etc more frustrating than the main missions.

You take a lot more damage in this difficulty, so start practicing your defibrillator QTE and ensure you always have Health infusions close to hand. Luckily, in NG+ you carry across all weapons, ammo, cash, components, Feltrite, MB cash and Project Points.

Preparation

Important Warning: If you have already finished the game, before starting NG+, open a previous save from your first run through (you may as well make it your last save - so you can keep all the items you collected), change your difficulty to Ultra Nightmare, then get the game to autosave. When you start NG+, you will be prompted to choose a save file to base your NG+ against - select the latest one, and make sure that difficulty is set there to "Ultra Nightmare". There are multiple reports that starting NG+ on a lower difficulty and then changing the difficulty once you start NG+ to Ultra Nightmare carries a risk of not validating the achievement once you finish the game the second time.

Note that you will need to have finished the main story and opened all 12 arks before you can start NG+. Visiting Arks again just give you weapon skins, so unless you feel the need for cosmetic upgrades, you can skip those.

If you hadn't already upgraded your health capacity to 150 via the Cyber Doctor in Wellspring, look to do that ASAP, potentially before you begin the NG+ run. You will also want to want to get your Damage buff up to the maximum (5 upgrades), and the Overdrive as high as you can (I was able to upgrade it 3 times out of 5.) You will also want your infusions and abilities fully upgraded.

Game Play

The game breaks down into roughly 3 parts:

Part I: Meeting Marshall, Loosum and Kvasir, and completing one mission from each.

Part II: Getting to level 5 in the Marshall Loosum and Kvasir projects.
Kvasir will likely happen naturally as you move through the map and come across data pads and ark chests, and any other icons with blue labels (such as fallen rangers.) Loosum requires you to complete orange locations, such as Automated Sentries, Gas Stations and Road Chokers. Marshall requires you to complete pink locations, which are mostly bandit dens. Level 5 for each person is 800 points, so you can pick harder targets for Marshall & Loosum to get to 800 with fewer locations that need to be explored, or if you find those too hard, do more of the easier locations.

Part III: Once you reach level 5 with each person, they will issue you one more mission each to complete. Once those are complete, you will then complete the final mission, Project Dagger.

You will want to return back to a Trade Town after each mission to restock on weapons/infusions/components. For the Part II section I found I could generally hit 2-3 locations before needing to restock (mostly for ammo).

Using the Phoenix to travel around is essential; at some of the bandit camps you can use the gatling gun and/or dual cannons to take a lot the enemies out while still in your car, then clean up the rest afterward. Note that your vehicle also takes significant damage in this difficulty.

Combat Tips

Your general combat approach will be around keeping your distance and strafing. Even Goons at close range can knock you down from 150 to 0 health in 2 or 3 shots. The Hyper Cannon becomes a defacto sniper rifle, and the Smart Rocket Launcher is also useful for taking out enemies at long range. Grav Darts are also useful and can take most (but not all) non-boss enemies in a single gravity sling shot, depending on the range.

The Slam, Vortex and Shatter abilities are also useful in various combat situations. The Slam is useful for groups of smaller, weaker enemies. Vortex is useful to give you a few moments of breathing room if you managed to capture several enemies in its pull. The Shatter ability is mostly useful for Goons who are holding shields, and in the final boss battle. At this difficulty level Shatter will not kill most enemies (unless they've been substantially weakened first.)

In Part III especially, you will begin to find Overdrive more useful. The Authority mutants that can fly/float that have energy weapons will rip you up really quickly, but the Charged Pulse Cannon + Overdrive takes care of them very quickly. Overdrive infusions even when upgraded only lasts 20-30 seconds but will definitely get you out of a jam.
